The WholeYouNYC Social Care Network is an exciting opportunity to contribute to a bold, community-driven initiative aimed at supporting social care services across the boroughs of Brooklyn, Queens, and Manhattan.
We invite you to apply through our open-ended Request for Applications (RFA) for SCN HRSN Service Providers to become a part of the transformative WholeYouNYC Social Care Network under the New York State 1115 Medicaid Waiver.
By joining this network, your organization can play a vital role in improving the health and well-being of New York City residents while also gaining access to funding and resources to expand your services.
About the WholeYouNYC Social Care Network
Since its inception, WholeYouNYC has rapidly expanded to include 300+ community-based service providers across Brooklyn, Queens, and Manhattan. These partners deliver more than 460 unique programs, serving over 1,100+ New Yorkers every month.
We believe that a connected system of care is the key to addressing health disparities and improving outcomes in our communities. Through WholeYouNYC, we are building reliable connections between healthcare providers, and community-based organizations. This collaboration ensures that individuals have access to the services and resources they need to lead healthier lives.
By joining WholeYouNYC, organizations gain access to a variety of resources designed to help them deliver effective and coordinated care. Our goal is to support each partner by making resources more accessible and creating a platform for community-wide improvement.
Who Can Apply?
We are looking for non-profit HRSN Service Providers to join our network. Eligible providers may include, but are not limited to:
- Community-based organizations providing services that address social needs, such as housing, food security, and transportation.
- Healthcare providers that offer wraparound services to patients addressing social determinants of health.
